INFORMED CONSENT

If you click the button entitled "Accept" below, your data will be saved as part of the National Health Survey and you will be enrolled in our research study. You must be at least 18 years old to participate. By clicking the "agree" button, you are agreeing to the following:

I understand that I am being presented with an interactive exercise log, but that I am under no obligation to enter any data. I understand that any information I provide may be used by the researchers for statistical purposes only. I understand that I can use this exercise log without providing my identity, but that if I provide my identity then the researchers will know the running, walking, and cycling distances I entered. I understand that my uncoded information will be stored on computer servers that lie behind a security firewall that are regularly monitored by Berkeley Laboratory computer security personnel. I understand that while there is a small chance that the confidentiality of the information could be compromised, the researchers will take care to prevent this from happening. I understand that my participation in this study is completely voluntary and that I am under no obligation to participate. I understand that the assessments of my physical activity represent how these data will be interpreted for this research project and that these assessments are not provided for the purpose of providing medical advice. I understand that I may not benefit directly from using this exercise log, but that society may benefit by the development of an effective tool for promoting physical activity. I understand that registration and entering my exercise data are expected to take about ten minutes. I also understand I may be recontacted at a future date to determine my interest in other studies. I understand that I can decline to participate in this or any future surveys or studies at any time without penalty. I understand that I can contact the researcher at health@lbl.gov if I have any questions about the study.
